docs(bf-4bhd): enumerate projection blob construction sites in report
Verified every line number in the blob-creation report against current main (5 primary types, 8 primary sites, 2 identity-mutation funcs, 9 projection types — all accurate). Added an enumerated table of the 15 projection construction sites across 7 files so the next bead needs no further searching. Completeness sweep confirmed no new(Blob), no missed factories, no uncatalogued make([]Blob) allocators. Co-Authored-By: Claude <noreply@anthropic.com>
